Certain cities in Alabama, Delaware, Kentucky, Maryland, Michigan, Missouri, New York, Ohio and Pennsylvania require an 8-part W-2 tax form. A typical 8-part set of W-2 tax forms includes Copy A for Social Security Administration, Copy 1 for state, city, or local tax department, Copy B for filing with employee’s federal tax return, Copy C employee’s records, Copy 2 for filing with the employee’s state, city, or local income tax return and Copy D for employer’s records.

We offer all these as one product in packages of 50. Designed as two filings per sheet, these forms are printing on laser or inkjet printers. The employer and employee sheets are separate to avoid confusion f any sort.
