The W-2 tax form is a Wage and Tax Statement used to report wages paid to employees and the taxes withheld from them. Employers must complete a Form W-2 for each employee to whom they pay compensation as part of an employer-employee relationship.

Displayed on this page is W-2 tax form in a 4-Up horizontal format. Each sheet has for four W-2s – Copy B & C 2 + 2 for sending to the recipient. Copy B is for filing with the employee’s federal tax return, Copy for the employee’s records and the other two for filing with the state, city or local tax department when needed.
